MPOWER MISSION TRIP POLICIES AND PROCEDURES

The $200.00 application fee submitted with your application is non refundable.

No one will be accepted as a team member until a completed application and deposit are received.

Your application will be reviewed and a personal interview may be required.

A mission trip can be rewarding and life changing, however, they can also be stressful. Please prayerfully consider factors in your personal life that may distract and prohibit you from fully committing to this trip and adapting to unusual conditions.

All trip costs are the team members responsibility and due 1 month before departure.

Participants must adhere to rules outlined in the team covenant.

If you are unable to participate in your trip for any reason, mPower must receive cancellation notice as soon as possible. You may be responsible for all trip costs.

Passport and vaccination costs are not included in the trip costs and are the responsibility of the team member. Team members assume the responsibility and liability for their personal health decisions.

A current passport is needed. If you do not have one, allow 12-14 weeks for this process.

Make sure all immunizations are current. We support the immunizations recommended by the Center for Disease Control. Many shots take 4-6 weeks to be effective, plan accordingly.

Medical Information & Release

This form must be filled out in order for you to participate in a Short-Term Mission Trip with mPower.

*Medical insurance is required for all short term mission trips.

In an emergency, I give my permission to a licensed physician to hospitalize or anesthetize me, or perform surgery on me. I understand that every effort will be made to inform my emergency contact before these actions are taken. 

Statement of Faith

1. We believe God wrote the Bible through men without error. (2 Timothy 3:16, 2 Peter 1:20-21)

2. We believe God exists eternally in trinity; Father, Son, and Holy Spirit. (Deuteronomy 6:4, 2 Corinthians 13:14)

3. We believe that Jesus Christ is the eternal God-man who became a man, (John 1:14) lived a sinless life (1 Peter 2:21-22), died on a cross, was buried and physically rose from the grave to redeem sinful humanity. (Acts 4:10, Philippians 2:7-11, Luke 23:26-24: 53)

4. We believe the Holy Spirit is the supernatural agent in salvation, placing all Christians into the universal church at conversion, indwelling and sealing them until the day of Christ's return. (John 16:8-11, 2 Corinthians 3:6, 1 Corinthians 12:12-14, Romans 8:9, Ephesians 5:18)

5. We believe all humans are sinful and in need of salvation. Salvation is the gift of God brought to humans by grace alone, and received by personal faith in the Lord Jesus Christ. (John 6:37-40, John 10:27-30, Romans 8:1, 1 Corinthians 1:4-8, 1 Peter 1:5)

6. We believe that Jesus is returning to Earth to judge all people and to rule and reign with His saints forever. (Revelation 22:12, 2 Timothy 4:1)

ADULT LEGAL RELEASE AND INDEMNIFICATION AGREEMENT

In consideration of the acceptance my participation in one or more mission trips and possibly including transportation by aircraft, bus, van, car or other form of transportation (collectively, the “Activity”) sponsored by mPower Foundation Inc. of Jefferson County, Kentucky, Inc. (“mPower”) and as an inducement to planning the Activity and permitting me to participate, the undersigned (hereinafter, “I”) agrees as follows:

I hereby fully and forever release and waive and agree not to bring or cause to be brought any and all claims, demands, actions, or causes of action of every possible kind and nature whatsoever I might assert, including, without limitation, claims for personal injury, wrongful death, or property damage, whether or not absolute, known or unknown, or otherwise against mPower or any of its board members, employees, representatives, agents and volunteers (collectively referred to herein as the "Releasees") by reason of, arising out of or relating to my participation in the Activity. I release the Releasees and waive all such claims against the Releasees for any injuries or damages caused in any manner whatsoever whether or not any such liability is attributable to the absence of ordinary or even slight care by the Releasees arising out of the Activity.

I further agree to indemnify, defend and hold the Releasees harmless from any kind of compensatory or punitive damages sought to be recovered from the Releasees and all losses sustained by the Releasees independent of claims asserted by others, including, without limitation, general damages, property damages, special, incidental and consequential damages, losses or expenses suffered or paid, directly or indirectly, as a result of any and all claims, causes of action, suits, proceedings, demands, judgments,
assessments, and liabilities, including reasonable attorneys' fees incurred in litigation or otherwise, assessed, incurred or sustained by or against the Releasees by reason of, arising out of or relating to my participation in the Activity.

I give mPower, and any person acting under its authority permission to publish, distribute, broadcast, televise, promote, license, sell and copyright for all purposes, any photograph, videotape, motion picture or sound recording produced directly by mPower or under its sponsorship, which may include my image or likeness and/or my voice. I also authorize mPower to reproduce, amplify, simulate, filter or otherwise distort my voice and all instrumental musical and other sound effects produced by me. In addition, I authorize the use of any printed material in connection therewith. I release all claims against mPower with respect to copyright ownership, publication and right to privacy. I agree that I shall not receive any fee and that all rights, title, and interest to the above materials and use of them belong to mPower. I understand and agree that these materials may be duplicated or distributed with or without charge, and/or altered in any form or manner without future/further compensation or liability, in perpetuity.

I further agree that I have read and understand this Agreement; I will not drive any type of motor vehicle while participating in the Activity; the execution of this Agreement is continuing in nature; it is my knowing and voluntary act; this Agreement is binding upon my heirs, executors, administrators and legal representatives; I am under no duress or undue influence to execute this Agreement; I represent that the information provided in my application to participate in the Activity is true, complete and correct in all
respects; it is my intention with this Agreement to make a complete, general, and unconditional release of any and all claims whatsoever against the Releasees as set forth above.

This Agreement shall be effective from the date signed below until the date of the receipt of a written declaration signed by the undersigned, that this Agreement shall no longer be effective with regard to any act or event from and after the date of the receipt via U.S. Mail of the written declaration addressed to mPower Foundation Inc of Jefferson County, Kentucky, Inc., 106 N. Watterson Trail, Louisville, KY 40243.

IN WITNESS WHEREOF, I hereby execute this Agreement on the date set forth below.

TEAM COVENANT

As a member of this team I agree to:

1. Remember that I am representing mPower and, more importantly, Jesus Christ. I will model Jesus in my behavior and attitude.

2. Remember that I am a guest working at the invitation of my hosts. I will remember the missionary’s prayer: “Where you lead me I will  follow. What you feed me I will swallow.”

3. Remember that I have come to learn as well as to teach. I’ll resist the temptation to inform our hosts about “how we do things.” I’ll be open to learning about other people’s methods and ideas.

4. Respect the hosts view of Christianity. I recognize that Christianity has many faces throughout the world, and that the purpose of this trip is to experience faith lived out in a new setting.

5. Develop and maintain a servant’s attitude toward all nationals and my teammates.

6. Respect my team leader(s) and his/her decisions.

7. Refrain from gossip.

8. Refrain from complaining. I know that travel can present numerous unexpected and undesired circumstances. Instead of whining and complaining, I’ll be creative and supportive.

9. Attend/participate in all team meetings before the trip as well as any follow-up meetings.

10.Remember not to be exclusive in my relationships. If my boyfriend/girlfriend or spouse is on the team, we will make every effort to interact with all the members of the team. If I am attracted to a teammate, I will not attempt to pursue a relationship until after we return home.

11. Refrain from any activity that could be construed as romantic interest in a national or
teammate.

12. I am not involved with illegal drugs and will abstain from consumption of alcoholic beverages and the use of tobacco while on this trip.

13. Observe and practice mPower’s Statement of Faith.

14. Refrain from the teaching or practice of any belief that would not be endorsed by mPower.

15. I will personally thank all financial donors that contribute for my trip.

16. Remember that I can be sent home if I do not adhere to this Covenant or if my Team Leader believes it is in my best interest of that of the team.

17. Refrain from driving any vehicle while out of the United States.
